Hip Steroid Injection

PREP: Refer to Lab Work Procedure Page to determine which drugs need to be discontinued and for how long, Pt. needs driver.
(Cell Count needs to be put in Lavender tube)
Dress patient in with all clothing removed from the waist down. Gown will open in back. Ask patient history questions and contrast consent form. Have them sign consent form. Position patient on back; using sheet to expose only hip area. Check order for steroid amount, but still check amount with Radiologist.
Myelogram sterile tray- add two 10cc syringes, 25 gauge 1 ½” needle, 18 gauge 1 ½“ needle. Get 40mg Depo Medrol and Marcaine out for Radiologist. Do not open.
10cc syringe: 9cc lidocaine & 1cc sodium bicarbonate
10cc syringe: 10cc omni 300 and attach long tubing (Dr. Lefler likes short tubing)
The other 10cc syringe and 18 gauge needle are for the Radiologist to draw up Depo Medrol and Marcaine. Typically the Radiologist injects 40 mg per cc for a total of 200mg of Depo Medrol and 3-4cc Marcaine 0.25% (bupivacaine). Ask Radiologist the amount of steroid they want, even if it is specified on doctor’s order. PLACE LEAD MARKER ON UNDERSIDE OF TUBE Let Radiologist mark patient. See arthrogram protocol for Radiologist marking and glove preference. Patient may walk to recovery room, please ask Radiologist how long to recover patient (some Radiologist let patient go right away).
